肠道准备与营养（四）|文献摘要（三）

直肠癌前切除患者在术前常存在不同程度的营养不良，经传统的肠道准备和手术应激，往往会造成营养不良加重。本研究将术前运用肠内营养粉剂安素做肠道准备的实验组，与传统术前行流质饮食做肠道准备的对照组进行对比，比较两组患者术前饥饿感、肠道清洁程度、术后肛门排气时间以及术后营养学指标。研究结果显示，实验组术前饥饿感患者比例低于对照组，术前灌肠次数低于对照组，术后肛门排气时间早于对照组。术后血红蛋白、血清白蛋白及血清前白蛋白术后水平，实验组的均值均有明显提高，说明术前口服安素粉进行肠道准备，能提高患者术后的营养状况。因此，口服安素代替传统流质饮食行术前肠道准备是一种安全有效的方法，不仅缓解术前饥饿感，保证肠道清洁度，还能促进胃肠功能恢复，提高患者术后的营养状态，值得临床推广。

摘要

目的：探讨肠内营养粉剂安素在直肠癌术前肠道准备中的应用效果。

方法：行直肠癌前切除术的患者50 例随机分为实验组和对照组，各25 例。实验组患者术前口服安素行肠道准备，对照组患者术前流质饮食。评估2 组患者术前饥饿感，记录术前灌肠次数，术后肛门排气时间、术后第1 天血清白蛋白、血清前白蛋和血红蛋白水平。

结果：实验组术前饥饿感患者比例低于对照组，术前灌肠次数低于对照组，术后肛门排气时间早于对照组，差异有统计学意义( P＜0.05) 。实验组术后第1天血清白蛋白、血清前白蛋白和血红蛋白水平高于对照组，差异有统计学意义( P＜0.05) 。

结论：术前口服安素粉行肠道准备是一种安全有效的方法，能缓解术前饥饿感，保证肠道清洁度，改善患者术后的营养状态。

Abstract

Objective: To investigate the application value of enteral nutritional powder （Ensure）in preoperative bowl preparation for patients with rectal carcinoma．

Methods:A total of 50 patients undergoing resection of rectal carcinoma were randomly divided into the study group and the control group，with 25 cases in each group．Patients in the study group were applied with enteral nutritional powder ( Ensure) in preoperative bowel preparation，and the patients in the control group were give liquid diet．The sense of hunger of patients before operation was observed. The enema frequency，time to anal exhaust，levels of serum albumin，prealbumin and hemoglobin at 1 day after operation were compared between two groups．

Results:The propotion of patiens with sense of hunger of patients was lower in the study group than that of controls (P＜0.05).Patients in the study group had lower enema frequency and earlier time to anal exhaust compared with those in the control group (P＜0.05)．Levels of serum albumin，prealbumin and hemoglobin at 1 day after operation were higher in the study group than those in the control group，with significant difference ( P＜0.05) ． 

Conclusions:Application of enteral nutritional powder (Ensure) in preoperative bowel preparation is effective and sate to alleviate hunger and improve the nutrition status of patient undergoing resection of rectal carcinoma．
