上廉，下廉，上下乃高低之意，位置所在，廉，此处有形状之意，此处肌肉凸起，形似镰刀，穴处刀之上下端，故名上下廉。
穴处一经，相似之处又有不同之别，都可以清热，上廉主泻胃热，清阳明二经实·热之症。下廉对大肠湿热，气机不畅较善攻之。
穴处相近，又都有近治作用，对于上肢麻痹，气机不畅，中风后的上肢功能受限均有帮助。
