驾驶台右侧门变形严重,关不紧。门橡胶穿孔。

30 EMCY GENERATOR STARTED FAILURE

EMCY发电机启动失败。

30 CABLE HOLE(20CM) ON BULKHEAD OF FORE BOSUN STORE CORRODED HEAVILY WITH HOLES

水手长物料间舱壁上的电缆孔(20厘米)严重腐蚀,有孔。

17 SHORT OF HANDRAIL IN CORRIDOR NEAR MESS ROOM

餐厅附近的走廊缺少扶手。

17 TWO WIPERS ON BRIDGE WINDOW OUT OF ORDER

驾驶台窗户上的两个雨刷坏了。

2.上海
2.1 PSC检查1

17 The actual rudder angle in steering gear room was 32 degrees when operating steering gear to hard port on bridge

当驾驶台操纵舵机至左满舵时,舵机室内的实际舵角为32度。

30 The emergency fire pump failed to start

应急消防泵启动失败。

30 Reserve source of MF/HF radio installation was defective

中频/高频无线电装置的备用电源有缺陷。

30 The isolation valve found malfunction as evidenced that a large amount of water spraying out from the poop deck fire hydrant when the isolation valve was shut-off and the main fire pump was running.

隔离阀发现故障,证据是当隔离阀关闭且主消防泵运行时,大量水从艉楼甲板消防栓喷出。

30 The fire damper of No.2 E/R ventilator can not be closed by remote manner

不能用遥控方式关闭2号机舱通风设备的防火挡板。

17 The life boat can not be started by battery marked with emergency switch

标有应急开关的电池不能启动救生艇。

30 One wire rope for launching the rescue boat broke during inspection

在检查过程中,一根用于释放救助艇的钢丝绳断裂。

右舷救生艇甲板登船梯的边绳腐烂并断裂。

17 The ship presented a danger that she unable to be maneuvered freely at NANCAO CHANNEL,area of high density and narrow water on 09th July 2023. It was happened that port anchor with chain dropped into water totally and could not be recovered due to port windlass motor damage

该船于2023年7月9日在南曹海峡出现了无法自由操纵的危险,该区域为高密度和狭窄水域。由于左舷锚机电机损坏,左舷锚链完全落入水中且无法回收。

2.2 PSC检查2

17 The fire drill is witnessed in E/R workship, it is found that two required jets of water are not supplied.

在E/R工作船见证了消防演习,发现没有提供两个所需的水柱。

17 SSB of MF/HF function testing failed when MF/HF was supplied by reserve power.

当MF/HF由备用电源供电时,MF/HF的SSB功能测试失败。

17 Both isolation valves malfunction, which is evidenced by a large amount of water spraying out from the fire hydrant on main deck under the circumstance of the shut-off the both isolation valves on poop deck and remaining water in fire pipe has been drained empty, as well as operation of No.2 main fire pump(Fire, Bilge & G/S pump) at the same time.

两个隔离阀都出现故障,在关闭艉楼甲板上的两个隔离阀的情况下,主甲板上的消防栓喷出大量水,消防管道中的剩余水已经排空,同时2号主消防泵(消防、舱底和G/S泵)也在运行。

17 No.1 main fire pump(Fire, Bilge & G/S pump) can not deliver at least two required jets of water.

1号主消防泵(消防泵、舱底泵和G/S泵)不能输送至少两股所需的水柱。

2.3 PSC检查3

30 The hydraulic oil spilled out heavily demonstrating the steering gear was unable to be worked properly, which can be a reason for the collision unexpectedly occurred at CJK channel on the date of inspection. And it is worth to be mentioned that two persons were lost in the subject collision

液压油大量溢出,表明舵机不能正常工作,这可能是检查当天在CJK海峡意外发生碰撞的一个原因。值得一提的是,有两人在这次碰撞中丧生。

30 Fixed water spraying for incinerator failed to be demonstrated at the time of inspection

用于焚烧炉的固定喷水系统在检查时演示失败。

30 One group of the quich closing valve in inoperable, and the subject group includes waste oil tank, G/E LO settle tank, M/E LO settle tank, cylinder oil store tank inoperable

一组快速关闭阀不可操作,该组包括废油柜、G/E LO沉降柜、M/E LO沉降柜、气缸储油柜等不可操作。

3.舟山

17 New installed generator engine (sn: 0414FWS) parameter record book not available

新安装的发电机发动机(sn:0414FWS),未能提供参数记录簿。

5名船员在船上服务超过12个月。

16 Fixed fire extinguishing system for independent generator engine not marked on fire control plan

独立发电机发动机的固定灭火系统未在消防控制平面图上标出。

30 The ventilation motor in independent generator engine not able to stop from outside

独立发电机中的通风电机不能从外部停止。

17 Lack spare foam for portable foam applicator in independent generator engine room

在机舱独立发电机室内的便携式泡沫,缺乏备用泡沫。

16 Tap in mess room not work

餐厅的水龙头坏了。

17 BNWAS key not controlled by captain

BNWAS钥匙不受船长控制。

安装在BNWAS控制面板上的复位功能不能正确保持了望。

小海哥提醒:这条缺陷是所有船的通病,都说等有人来检查再关,最后往往忘记关。

左舷的一个生活区通风孔严重腐蚀,左舷的一个通风孔盖缺少密封垫。

17 Several lifebuoys on aft/main deck not in place and the storage position not posted IMO symbol.

船尾/主甲板上的几个救生圈没有到位,存放位置没有张贴IMO标志。

17 The paper charts(3026/344/346) not new edition.

纸海图(3026/344/346)不是新版。

油漆间的喷水系统故障,没有安装喷水器,水压不足。

30 Fire main found water leaking seriously

消防总管发现漏水严重。

30 The closing devices of two fresh water tank air pipes and steering gear room ventilation found seized, and the cover of port side no.2 ballast ballast water tank corroded and holed.

发现两个淡水舱气管的关闭装置和舵机舱通风被卡住,左舷2号压载水舱的盖子被腐蚀并有孔。

日光信号灯开关故障

30 The damper of engine room funnel ventilation cannot be closed

机舱烟囱通风的风门不能关闭。

30 The deficiencies marked with ISM related indicated that the SMS was not implemented effectively onboard as required by ISM/10. Additional audit shall be carried out.

标有ISM相关的缺陷表明,安全管理体系没有按照ISM/10的要求在船上有效实施。应进行额外的审核。

17 The forward winch found oil leakage

船首绞缆机车发现漏油。

船上未提供救生艇的5年检验报告。

17 The cargo hold bilge alarm in ship office not working

在办公室的货舱污水报警器不工作。

17 The drain plug not attached to lifeboat

艇底塞未连接到救生艇上。

16 the height of two ventilators of gally exceeds 90mm, but without specially support.
厨房的两个通风口的高度超过90毫米,但没有特别的支撑。

30 B-Deck entrance door fell off, the self-closing device broken.

B层入口门脱落,自动关闭装置损坏。

30 The fixed CO2 fire extinguishing system pipeline for cargo hold seriously corroded and holed.

货舱固定式二氧化碳灭火系统管线严重腐蚀穿孔。

30 The ship air conditioner compressor malfunction.

船舶空调压缩机故障。

30 The isolation valve of No.1 Generator malfunction cannot be closed remotely.

1号发电机故障隔离阀不能遥控关闭。

16 The arrangement of the bilge draining system of bosun store cannot discharge all the bilge water remotely.

水手长物料间的舱底排水系统的布置不能远程排放所有的舱底水。

4.天津

30 THE ENGINE OF FREEFALL LIFEBOAT CANNOT BE STARTED BY BATTERY NO.1 (NO MEANS OF MANUAL STARTING SYSTEM ON BOARD)

自由降落救生艇的发动机不能由1号电池启动(船上没有手动启动系统)。

30 FIRE DRILL DID NOT COMPLY WITH THE REQUIREMENT OF SOLAS, DETAILS AS FOLLOW: 1. TWO SETS OF FIRE-FIGHTER'S OUTFITS WERE NOT WORN CORRECTLY. 2. NO ONE PROPERLY CHECKED FIRE-FIGHTER'S OUTFITS BEFORE WEARING THEM. 3. NO ONE CHECKED THE RELEVANT COMMUNICATION EQUIPMENT BEFORE ENTERING THE FIRE SITE. 4. WHEN FIRE-FIGHTERS ENTERED THE FIRE SITE, LIFELINE WAS NOT BE ATTACHED TO THE SNAP-HOOK OF HARNESS OR BELT, BUT HELD IT IN HIS HAND. DURING THE FIRE DRILL, THE LIFELINE WAS LOST IN STEERING GEAR ROOM. 5. BEING THE SCENE COMMANDER, CHIEF MATE ENTERED THE FIRE WITHOUT WEARING FIRE-FIGHTER'S OUTFIT.

消防演习不符合SOLAS的要求,详情如下:1.两套消防探火服没有正确穿戴。2.没有人在穿消防服装之前仔细检查过。3.没有人在进入火场前检查过相关的通讯设备。4.当消防队员进入火场时,救生索不是系在安全带或腰带的弹簧钩上,而是拿在手里。在消防演习中,救生索在操舵室丢失了。5.作为现场指挥官,大副没有穿消防员装备就进入了火场。

17 ABANDON SHIP DRILL DIE NOT COMPLY WITH THE REQUIREMENT OF SOLAS, DETAILS AS FOLLOWS: 1. ELEVEN PERSONS (INCLUDING CHIEF OFFICER) DID NOT WEAR LIFE-JACKETS CORRECTLY. 2. NO ONE CHECKED IF LIFE-JACKETS WERE WORN CORRECLY, BUT CHIEF MATE REPORTED TO CAPTAIN THAT ALL CREW ARRIVED AND LIFE-JACKETS WERE WORN CORRECTLY.

弃船演习不符合SOLAS要求,详情如下:1.11个人(包括大副)没有正确穿救生衣。2.没有人检查救生衣是否穿对了,但是大副向船长报告所有船员都到了,救生衣穿对了。

17 PART OF STBD SIDE RAILS NEARBY CARGO HOLD NO.2 ON UPPER DECK DEFORMED.

在2号货舱附近的上层甲板上的右舷纵梁部分变形。

17 LIQUID LEVEL GAUGE OF EMERGENCY GENERATOR COOLING WATER TAK IN DIRTY, LIQUID LEVEL OF ABOVE TANK INVISIBLE.

应急发电机冷却水柜的液位计脏污,上面水箱的液位看不见。

17 NO RAT GUARD WAS FIXED ON TWO STERN SPRING LINES.

两条艉倒缆上没有安放防鼠板。

17 AMMETER RED POINTER INDICATED THE ALARM CURRENT VALUE OF NO.4 E/R SUPP./EXH. FAN IS IN THE WRONG POSITION.

安培计红色指针指示4号机舱风机的报警电流值的位置不对。

30 The emergency generator failed to start when simulating ship black-out testing

模拟船舶停电试验时,应急发电机未能启动。

30 The remote quick closing devices for M/E NO.1 & NO.2 SERV. tank and G/E D.O. SETT. tank & L.O. SERV. tank & CYL. oil tank malfunction

用于1号和2号主机日用柜和G/E D.O沉淀柜,滑油日用柜,汽缸油柜等的遥控快速关闭装置故障。

17 No duty persons are designated to keep watch near the anchor equipment when the ship is sailing under anchor stand-by situation in NANCAO CHANNEL. Therefore, when the anchor was out of control, no immediate actions can be taken to process this.

船舶在南曹海峡锚泊状态下航行时,锚设备附近无值班人员值班。因此,当锚失去控制时,不能立即采取行动来处理这种情况。

17 The ship presented a danger that she was unable to be manneuvered freedly at NANCAO CHANAEL on the 28th September 2023. It happened tha the port anchor suddenly was out of control and dropped into the Nancao channel, which further caused the damage of the hydraulic driving motor of portside wndlass

2023年9月28日,该船出现了无法在南曹海峡自由操纵的危险。左舷锚突然失控,掉进南曹海峡,进一步造成左舷锚机液压驱动马达损坏。

17 The emergency bilge suction valve is difficult to open

应急舱底吸入阀很难打开。

30 The ship discharged 0.02 cubic meters unground food waste into the sea where is about 10.69nm from Chinese baseline on 30th May 2023

该船于2023年5月30日,在距中国基线约10.69海里的海域排放了0.02立方米未磨碎的食物垃圾。

30 Four sets of air conditioners with ODS (R22) as freezing agent were intalled on this vessel on 28th Feb. 2022 and are in normal use since then. And these four sets of air conditioners are not listed in the supplement to IAPP
