背景:结直肠手术后肠麻痹仍是外科医生所面临的难题,延迟患者的康复并增加其住院时间和费用。
目的:本研究旨在充分、有效地实施加速康复背景下,分析围手术期液体管理对结直肠手术后肠麻痹发生的影响。
设计:这是一项基于前瞻性登记的机构数据库的回顾性队列研究。
设定:本研究是在三级教学医院施行的,在整个研究期间已全面实施标准化的加速康复路径。
患者:本研究纳入2011年-2016年间连续收治的、患有良性或恶性肿瘤而接受结直肠切除手术的所有患者。
主要结局指标:术后肠麻痹的定义为需要重置鼻胃管。本研究比较了有和无肠麻痹患者的围手术期液体管理和手术学结果。通过多因素Logistic回归分析法判断影响患者肠麻痹的潜在的危险因素。
结果:该研究共纳入4205名患者,共有377名(9%)在术后第4天(四分位间距为2-5天)发生肠麻痹。肠麻痹患者术中输液量为3.2±2.6 L,非肠麻痹患者的输液量为2.5±1.7 L(p<0.001)。术后第1天,肠麻痹患者的体重增加了3.8±7.1 kg,非肠麻痹患者的为3.0±6.6kg(p = 0.272);术后第2天,肠麻痹患者的体重增加了4.4±6.5 kg,非肠麻痹患者的为3.1±7.0 kg(p = 0.028);出院时两组患者的体重增长分别为1.8±6.0 kg和0.0±6.0 kg(p = 0.002)。将所有有显著性差异(p <0.05)的人口学指标、液体管理相关指标和手术学参数进行多因素分析后发现:术后当天的输液量> 3 L(OR = 1.65(95%CI,1.13-2.41); p =0.009)、术后第2天体重增加> 2.5 kg(OR = 1.49(95%CI,1.01-2.21); p = 0.048)和术后并发症的发生(OR = 2.00(95%CI,1.39-2.90); p <0.001)是发生术后肠麻痹的独立危险因素。
局限性:本研究受限于回顾性研究的属性。输液管理取决于与患者、疾病和手术相关的多种因素,因此不能一概而论和随意外推。
结论:液体超负荷和术后并发症的发生是发生术后肠麻痹的独立危险因素。这需要采取措施,以使围手术期输液量低于结果中描述的临界量。
结直肠手术后发生的术后肠麻痹(POI)仍然是一个重要问题,它延迟了患者的康复并增加了住院时间和费用[1-2]。已有多种通过随机对照试验进行验证的预防POI的措施,包括泛影葡胺经口给药[3-4]、爱维莫潘(译者注:alvimopan,一种新型的口服外周阿片样受体拮抗剂)[5]、口香糖[6-7]或硬膜外镇痛[8-9],但结果并不一致或仍不明确。根据既往和最近的报道,围手术期限制性液体输入可能是一种预防POI的方法[10-11]。但是应当谨慎,因为过度地补液限制可能会导致急性肾损伤(AKI)[12]。本研究旨在评估结直肠手术围手术期加速康复路径(Enhanced recovery pathway,ERP)中进行的液体管理对术后POI发生的影响。
患者和方法
这是一项基于梅奥诊所机构电子ERP数据库中前瞻性登记的数据而进行的回顾性队列研究。研究纳入2011年至2016年间连续收治的、因良性或恶性肿瘤接受择期结直肠切除术[包括左半、右半、(次)全、节段性结肠切除术和直肠切除术)]的患者。本研究排除了经肛局部切除术、造口术(造口闭合术和Hartmann还纳术)以及疝修补术。该研究作为加速康复质量改进项目的一部分,已通过伦理委员会的批准。
本院结直肠外科于2009年11月开始启动ERP项目、于2010年开始实施,并于2011年1月形成实践标准。在先前发表的机构出版物中已多次描述过该数据库的内容,这些文献证实ERP实施过程中依从性高,并能改善的短期疗效[13-16]。简而言之,该实践标准包括基于多模式的、阿片类药物限制式(即尽量避免阿片类的使用)的镇痛策略(只用腰麻、而不用硬膜外麻醉)[16-17]、术后恶心和呕吐(PONV)的预防(加巴喷丁全身给药)、限制性术中输液管理以及早期恢复正常饮食和活动,这些都与美国加速康复协会的建议相符合[18-19]。在麻醉苏醒前我们常规拔除鼻胃管(NGT)。
液体管理
在ERP实践标准设计中,我们采用限制性的术中输液管理策略以实现术中输液出入平衡。围手术期管理理念还包括在麻醉诱导前2个小时允许口服流质[18]。术后患者可不受限制地口服液体(如能耐受)。遵循的原则与美国加速康复协会的建议相符[18]。在血容量正常的患者中,使用血管加压药物维持血压以避免液体过多。单独来说,术中少尿并不被认为是有害的,也不用于指导液体复苏。根据麻醉师的评估,于术中针对高危患者采用侵入性的监测和目标导向的液体管理策略,首选通过有创血压监测和/或经食道多普勒技术对心搏量进行评估[20]。把术中、麻醉后监护病房内和术后第1天(POD 1)上午8:00前在外科病房内的所有静脉补液量(优选等渗的氯化物限制性晶体、胶体、浓缩红细胞和白蛋白)进行相加,作为手术当天(POD 0)的静脉补液量。收集的其他数据还包括术后第2天的静脉补液量和口服液体摄入量。
分别测量手术前(接受手术前1个月)、术后第1天(POD 1)、术后第2天(POD 2)和出院时的体重。基于本研究的目的,本研究设定2个的液体临界值:手术当天(POD 0)静脉输液量的为3 L,术后第2天(POD 2)体重增加的为2.5 kg。这些液体管理临界值提出于另一个独立的研究队列,并且首次应用于本研究进行外部验证[21]。
结局
POI定义为在住院期间或术后30天内需要重置NGT[22]。记录了重置NGT的时间点。首先,在患者相关的、手术相关的和输液管理相关的各种参数中判定影响POI发生的危险因素。第二步,纳入单因素分析中具有显著性的危险因素进行多因素分析,以探讨上述的液体管理相关临界值对POI发生的影响。评估并比较POI及无POI两组患者在住院期间及术后30天内发生的总的和特定的手术并发症、再手术率及再入院率的差异,并发症包括:出血(需要输血的)、需要有创治疗(手术、经皮或伤口负压治疗)的手术部位感染(SSI)、吻合口漏(临床或影像学证实)、急性肾损伤(AKI)(定义为术后肌酐升高达1.5倍);记录POI的患者需再手术的原因。本研究仅分析初次手术数据,根据意向性治疗的原则,本研究未排除任何患者。当发生再手术时,由于再手术为非择期手术,在再次外科干预后不再对POI进行评估。
统计学分析
在描述性统计中,对于分类变量使用频率(百分比)进行报告,对于连续变量根据情况采用均数(SD)或中位数(四分位间距(IQR)进行报告。χ2检验用于比较分类变量。所有统计学检验均为双侧,以0.05表示差异具有统计学意义。将有显著性差异的变量纳入多因素Logistic回归分析(基于概率回归模型),以估算比值比(OR)。使用SPSS Advanced Statistics 22软件(IBM软件集团,Chicago, IL)进行数据分析。
结果
4205名患者中共有377名(9%)在术后第4天(四分位间距为2-5天)发生了肠麻痹。人口学和手术学细节资料见表1和表2中。根据手术类型,左半结肠切除术后肠麻痹发生率为6.3%,右半结肠切除术后的为8.6%,直肠手术后的为10.0%,节段性/横结肠切除术后的为11.2%,全结肠切除术后的为11.4%(p = 0.05)。

POI患者和无POI患者两组间在术后当天直至术后第2天的静脉输液量和口服液体摄入量具有显著性差异。在术中,POI患者的输入量为3.16±2.55 L,对照组的为2.48±1.65 L(p<0.001)。当对术后当天(POD 0)的输液总量进行逐年评估时,未观察到术中液体治疗模式(输液总量)随年份而增加或减少(2011年:3.2±1.6 L,2012年:3.2±1.6 L,2013年:3.2±1.9L,2014年:3.1±2.2 L,2015年:3.3±1.9 L,2016年:3.2±1.8 L)。在POD 0,输入了大于3 L液体量在POI组有236例(63%),而在无POI组中为1683例(44%)(p <0.001)。同样,两组间POD 2和出院时的体重增加也存在差异:无POI患者出院前恢复到初始体重,而POI患者的体重增加1.8 ± 6.0 kg(p = 0.002)。POD 2体重增加>2.5 kg的情况在POI患者中更为常见(68%vs. 56%,p = 0.008)。术后静脉输液和口服液体摄入量如图1所示。两组平均口服液体摄入量在POD 0分别为6.0 ± 5.5比 6.0±4.8 mL / kg(p = 0.944),在POD 1的分别为16.8 ± 9.1比 18.2±8.6 mL / kg(p = 0.021),在POD 2的分别为13.1±8.9 比15.7±8.8 mL / kg(p <0.001)。

表3比较两组的总体和特定术后并发症(除POI)情况。吻合口漏、SSI和再手术的中位发生时间分别是术后第5(IQR,3-8)、9(IQR,4-15)和3(IQR,1-10)天。两组的中位住院时间存在显著性差异(POI患者的为9天,无POI患者为4 天;p <0.001)。POI患者再次手术的原因有:切口问题,包括浅表SSI和切口/腹壁裂开(35%)、腹腔内脓毒血症包括吻合口漏(28%)、小肠麻痹(17%)和出血(14%)及其它(6%)等并发症。
