This case, what's the most likely diagnosis? So let's describe the case. Large cystic mass, head of the pancreas, multiple septations, pushing on the portal vein but not obstructing it. 

Now SPEN tumors usually aren't in 55 year old, and they usually more than half of calcification. Mucinous cystic neoplasms, the age is good but that's usually the body and don't have septations typically. It's usually just a cystic lesion.

 There endocrine tumors can be vascular typically, but they can be cystic there in the tumors, but they typically have rim like enhancement, and typically not as many septations. 

This is a serous cystadenoma. I know sometimes serous cystadenomas look more aggressive. Here you see some compression of the portal vein, a little bit of scalloping, as important to recognize that indeed can occur. It's important to recognize as serous cystadenomas can cause pancreatic duct obstruction, and at times dilated common duct. But serous cystadenoma  are a tumor of many many shapes.

这个病例最可能的诊断是什么呢？我们先来描述一下：胰头巨大囊性肿块，其内多发分隔，肿块挤压门静脉但未引起梗阻。

实性乳头状上皮肿瘤不常见于55岁，并且通常超过半数伴钙化。粘液性囊性肿瘤，年龄符合，但常位于胰体，典型的病变没有分隔，常仅呈囊样灶。

神经内分泌肿瘤通常富血供，但肿瘤呈囊性，典型的表现为轮状强化，内部没有那么多分隔。

这是一例浆液性囊腺瘤。有时候该病变侵犯性更强。这里可见门静脉受压迫，呈轻度扇状，需要注意该征象常会出现。同样也需要注意浆液性囊腺瘤会引起胰管梗阻，有时引起主胰管扩张。但是（实际上）浆液性囊腺瘤可以呈现多种表现。
