248.Invasive aspergillosis （ 侵袭性曲霉菌病）

Diagnosis:  Invasive aspergillosis.

【Invasive aspergillosis 侵袭性曲霉菌病】

Differential:  Angioinvasive fungal disease, necrotizing bacterial pneumonia, septic emboli. 

诊断：侵袭性曲霉菌病。

Discussion:

Invasive pulmonary aspergillosis is a life threatening infection most commonly affecting patients with severe neutropenia related to leukemia, chemotherapy, bone marrow, or solid organ transplantation.  The risk is greatest when the neutrophil count is below 500 cells/mm3.  Chest radiographs may be initially normal, and with time focal or multifocal nodular or lobar consolidation may develop.  

鉴别诊断：侵袭性真菌感染、坏死性细菌性肺炎、脓毒性栓子。
讨论：侵袭性肺曲霉菌病是一种威胁生命的感染性疾病，常累及中性粒细胞减少的患者，如：白血病、化疗、骨髓或实性器官移植。当中性粒细胞计数低于500的时候，风险最高。常规胸片开始可无异常，随病情进展，可出现局灶性、多发性结节或肺叶的实变。
