Living Well With Chronic Conditions

Living Well with Chronic Conditions is a six-week workshop that provides tools for living a healthy life with chronic health conditions, including diabetes, cancer, arthritis, asthma and heart disease.

Workshops related to Living Well that are also available in communities across Oregon include Tomando Control de su Salud, a Spanish-language, culturally appropriate version, and the Positive Self-Management Program (PSMP), a workshop for people with HIV.

Oregon Tobacco Quit Line

The Oregon Tobacco Quit Line is a free program offering tips, information, and one-on-one telephone counseling to anyone looking to quit tobacco or help someone quit.  Available to all Oregonians regardless of income or insurance status, the Quit Line is staffed by real people who are friendly and non-judgmental.

Quit coaches will help you make your own plan to fit your own life. No pressure, no guilt. Coaches can even help you figure out if you are eligible for free nicotine gum or the patch.

Open 7 days a week, 4:00 am to 12:00 am. Coaching is available in many languages.

Oregon Arthritis Program
Arthritis Foundation Exercise Program (AFEP)

The Arthritis Foundation Exercise Program is a safe exercise program for anybody wanting to start a beginner's fitness class or to maintain muscle strength. The program uses gentle activities to help increase join flexibility and stamina and has been proven to decrease pain in people with arthritis. Exercises can be done standing or seated in a chair.

Walk With Ease Program (WWE)

Walk With Ease is a six-week walking program conducted in groups by a trained leader. Participants receive health education tips, learn how to exercise safely and create a personal walking plan that fits their goals. Each session includes a lecturette, warm up, 10-30 minute walk and cool down. Participants receive a textbook and track their progress in a walking diary.

EnhanceFitness Program

The EnhanceFitness (EF) program is a multi-component group exercise program designed to increase strength, improve flexibility and balance, boost activity levels, and elevate mood. EF includes a five-minute warm-up, 20-minute aerobics workout, five-minute cool-down, 20-minute strength training and 10-minute stretching. EF is offered to groups of 10 to 25 participants as one-hour classes that meet three times per week on an ongoing basis.

National Diabetes Prevention Program

The National Diabetes Prevention Program is a community-based lifestyle change program designed to prevent type 2 diabetes in adults who have prediabetes or are at risk for developing diabetes. Participants work with a Lifestyle Coach in a group setting that includes 16 weekly sessions over a six month period, followed by six or more monthly follow-up sessions. During the year-long program the Lifestyle Coach and program participants work together on eating healthy, increasing physical activity, and overcoming barriers to living a healthy lifestyle.

Diabetes Self-Management Education (DSME) Programs
DSME programs help people with diabetes gain the knowledge and skills needed to modify their behavior and successfully self-manage the disease and its related conditions. Learn more and find local programs through the following links.

    
Falls Prevention for Older Adults
Tai Chi: Moving for Better Balance (TCMBB)

Tai Chi: Moving for Better Balance is an eight-form version of Tai Chi that was designed to improve strength and balance while reducing fear and risk of falls. Participants attend one hour classes 2-3 times per week for three months.

Self-Management Webinar Series

This quarterly web-based training is available for individuals who are currently implementing one or more of the above programs or who would like to learn how to get started.
